Medicare
Plus and Enhanced Primary Care Plans
Claire and other Dietitians employed by the practice are registered
with the Health Insurance Commission and available to work with
medical practitioners on Enhanced Primary Care Plans, under the
new Medicare Plus initiative introduced in July 2004.
We can assess your patient's current nutritional status and provide
medical nutrition therapy and education of patients, carers and
families. You will be sent a comprehensive report on your patients
progress after each consultation.

can assist you to improve patient outcomes:

Either
your patient or you can phone Claire on 03 9819 4414. Enquiries about
appointments can also be made by email.
