Medicare Chronic Disease Management Plans
All EatWell Dietitians are registered with the Health Insurance
Commission and DVA and are available to participate in multidisciplinary teams on Chronic
Disease Management Primary Care Plans for patients in Kew and surrounding suburbs Melbourne. Our Dietitians can assess your patient's current
nutritional status and provide medical nutrition therapy and education of
patients, carers and families. You will be sent a report on your patients
progress after the initial and final consultation.

To refer a client, you or your client can call us on 03 9853 4910 or complete our online booking form (Claire will then call the client back to arrange an appointment time with one of our Dietitians).
